What are the insurance requirements?

Community providers must have a minimum of $1,000,000 in liability insurance / $2,000,000 General Aggregate insurance with iLEAD listed as Additional Insured, a Waiver of Subrogation, and a Primary and Noncontributory endorsement in favor of iLEAD.

Note: High-risk community providers may have additional insurance requirements.
